If you are unable to keep your appointment, please let Cheri know as soon as possible by phone or text at 928.707.0761. Every effort is made to keep appointments on schedule out of respect for all. Emergencies do arise, however, and it is recognized that sometimes a delay is unavoidable.

Please check with your insurance provider prior to treatment to ensure that you are covered for your treatment and to be sure that they are listed as a provider in your plan. Session payments may be applicable toward your out-of-network deductible. It is the client's responsibility to check their insurance plan coverage and to know the specific requirements of their coverage. If you change insurance companies or change your address, phone number, etc., please let them know as soon as possible so necessary changes to your file can be made.
Private pay is accepted in cash, check, credit card and through Health Savings Accounts (HSA). A sliding fee schedule is available upon agreement.
Intake Process:
After your initial contact with Cheri, you will be provided with a link to the portal for Alma or Headway websites to complete intake paperwork. A link to the telehealth website, Doxy.me, will also be sent. These are all HIPAA compliant platforms.
